VIOLETTE VISIONS EARLY LEARNING CENTER CHILDREN WITH DISABILITIES
If a parent wishes to enroll a child with any disability, or we suspect a need for evaluation or assistance for a currently enrolled child, we discuss the need with the parents. Parents of children under three years of age are informed of the Early Intervention Program at U-Mass Hospital. Parents of children age three and up are notified of their rights under chapter 766 for Special Education/Evaluation. With the written permission of the parent we will request information from these sources which relate to the child’s participation in our program. Documentation of any referral will be documented in the child’s file. The child’s parents, teachers, center director, and the federal agency will work together to determine whether or not we will be able to meet the child’s needs at our center. Any individual plan for that child’s education will be signed by all parties involved and will be maintained in the child’s file. The plan would be reviewed every three months and records of the child’s development, behavior, improvements, etc. would be added to the child’s file. The individual program plan shall identify the specific services required to meet the needs of the child as well as any changes or limitations applying to the child’s participation in center activities. Specific services might include: Physical Therapy, Speech and Language Therapy, Psychological or Psychiatric services, Educational services, Social services, or Occupational Therapy. The plan shall identify the methods by which the above services would be provided, such as: who will provide the service, a schedule for provision of services, where the service will be provided, any special equipment, ramps, materials, or aides required for the child. The plan shall recommend the size of the group to which the child may be assigned, appropriate staff / child ratios, and any necessary modifications. In the event that it is decided that we are unable to meet the child’s needs, we will provide written notifications to the parents. This will occur within thirty days of the receipt of authorized, requested information and will include reasons for the decision. The notification shall inform parents that they may contact the OFC and request and the office determine that the child’s needs could be met without undue burden to the center. Parental permission in writing will be obtained prior to any discussion of the child between the center and any referral agency / educational agency, or the center’s involvement in the child’s individual program plan. The director or assistant will act as the liaison for any child with a disability. She will be responsible for coordinating care in the program and with service providers. Signed parent authorization / consent will be valid for one year unless changed by the parent prior to that.
