VIOLETTE VISIONS EARLY LEARNING CENTER PARENT AGREEMENT
The center is closed on New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day & Day After Thanksgiving, Christmas Day, (and the day before or after Christmas & July 4th when these holidays fall on a Tuesday or Thursday). When a holiday falls on a weekend, the legal holiday is that Friday or Monday. Emergency closing information~ call the center at 508-845-3248- we do not close for snow but do not guarantee to open on time. Expect a 1-2 hour delay based on Shrewsbury Public school closings~ call first. Tuition remains the same during holiday weeks; no discount is given for these closings. Tuition is due on the first scheduled day of attendance each week. Late fees are charged. Late payment fee: $10.00 weekly for each week owed. Tuitions that are two or more weeks in arrears may result in temporary dis-enrollment. Tuition remains the same even when a child is absent for one or more days in a week. A reservation fee of half tuition is due when a child is absent for a full week or more. Registration and program fees are due upon enrollment and yearly (spring re-registration) thereafter. Returned check fee: $25.00. Late pick up fee after 6:00 pm: $1.00 per minute per child. I agree to give the director a one-week notice of intent to withdraw my child or pay the extra week's tuition. I will be given a one-week notice if the center finds it necessary to dis-enroll my child. I understand that I must provide documentation from my child's doctor showing all required immunizations including lead and TB testing and a yearly checkup. This information must be updated yearly and I will provide an appointment card specifying the date of the scheduled physical if one cannot be completed by the date of expiration. Parents are required to sign off that they have read the following center policies: HEALTH CARE, STAY AT HOME, EMERGENCY INFORMATION, CHILD ABUSE & NEGLECT, STATEMENT OF PURPOSE & GOALS, ORGANIZATIONAL INFORMATION, REFERRALS, CHILDREN WITH DISABILITIES, BEHAVIOR MANAGEMENT, TERMINATION & SUSPENSION TRANSPORTATION, PARENTAL RIGHTS, INPUT & VISITS, TOILETING & DIAPERING PROCEDURES, EDUCATIONAL PROGRAMS, AND DAILY ACTIVITY SCHEDULES. I understand that I am free to visit anytime with or without notice. I am to sign my child in & out at the front desk and walk my child to the teacher and be certain that my child's arrival or departure has been acknowledged. I understand that no food or drink is to be brought to the center without the center's approval. The only reasons for bringing food to the center are infant feeding requirements, medical or religious requirements, and birthdays or other special events. Vegetarian diets may be arranged. I understand that toys from home should not be brought to school. Each classroom has its own policy on SHOW AND TELL, which may include toys from home. I will check with my child’s teacher to inform myself of their policy. NO VIOLENT TOYS such as guns or knives may be brought to the center. Special dolls, stuffed animals, blanket, etc. for naptime are allowed daily. The center is not responsible for toys or other items, which are brought to the center. I agree to keep a full change of clothing, including socks and underwear in my child’s cubby. INFANTS: The parent shall provide a written plan for feeding and change / update the plan as needed. The staff will provide a detailed report of activity each day. The parent shall provide: formula, baby food and juice until the infant may have table food, milk, etc. which is provided by the center. The parent shall provide: diapers, wipes, creams, lotions, powders, bottles, pacifiers, two full changes of clothing, and any other necessary items. The parent will reimburse the center for any such supplies, which must be purchased, if the parent fails to bring them. Any unused portion of food or formula will be returned to the parent or discarded at the end of the day in accordance with state regulations. Parental involvement: We ask that parents participate in the following ways: attending school events, donating materials, special snacks or recipes, my time, knowledge or expertise of occupation, hobbies, talents, etc. |
