SOCIAL SERVICE REFERRAL PLAN

If a child shows signs of having problems of any kind, staff members are required to notify the director or assistant director, who will act liaison for any referral. She will assist the parent and/ or teacher in looking for help or documenting behavior. The teacher assigned to the child will be responsible for documenting all evidence of an existing problem and recording any unusual behavior or developments. After discussing the problem with teacher, the director will decide if the parent needs to be called in for a conference. After obtaining written permission from the parent, the liaison will recommend that the child see a specialist if that appears necessary, and the liaison will assist the parent in finding such help. Referrals to appropriate social, mental health, educational, and medical services may include but are not limited to referrals for dental checkup, vision screening, and hearing screening.

All behavior, conversations with parents, and / or recommendations will be documented and maintained in the child’s file. The liaison or director will follow up on recommendations by asking what action has been taken and what the results were. This information will be added to the initial report. This assures that the situation is being handled by experts, and allows the parent an opportunity to ask for additional help in solving the problem.

If a child’s behavior indicates a need for special assistance, the parent will be notified, during the initial conference, of their rights under chapter 766 of the acts of 1972 and its regulations. If the child is under three years of age the parent will be notified of the Early Intervention Program at U-Mass Hospital. Upon written request of the parent, the child will be referred to an appropriate agency. Depending on the results, and after recommendations have been made, a determination will be made as to whether the child’s need can be met at our center. Any efforts and / or accommodations made by the center will be documented and added to the initial report. With written permission from the parent, the director of liaison will contact the agency or service provider who evaluated the child for consultation and assistance in meeting the child’s needs. Any individualized plan for the education of the child will be maintained in the child’s file along with the initial documentation and referral information. The child’s progress will be reviewed every three months, and will be recorded in the child’s file.

In the event that we are unable to meet the child’s needs, in the opinion of the center, parent, and / or referral agency, the child may be terminated from the center.

All behavior which was observed, all recommendations made, conversations held, and all concerns, actions, and results will be documented and maintained in the child’s file.
